To retrieve your Username:

1. To retrieve your username, enter in the email address that is associated with your Extranet Account into the space below.
   Please NOTE: if you do not know or have access to the email account associated with your username, please contact the Extranet Help Desk at extranet@azftf.gov
2. Click Send Username. Only click this button ONCE.
